چکیده

270 We present a case of a 61 ‐year ‐old man with a his‐ tory of pancreatitis, who presented at the emer‐ gency department with chest pain, vomiting, and dysphagia. His blood count, the levels of cardi‐ ac and liver enzymes and electrolytes, as well as the results of renal function tests were normal. An electrocardiogram revealed sinus rhythm with‐ out any changes of ischemia. Chest X ‐ray with contrast showed restriction in the distal esoph‐ agus without infiltration of the wall (FIGURE 1A). Endoscopy showed compression of the distal esophagus by an extrinsic mass (FIGURE 1B). Com‐ puted tomography (CT) scans of the back medi‐ astinum revealed a pseudocyst extending from the abdomen to the chest through the hiatus of the esophagus (FIGURE 1C). Endoscopic retro‐ grade cholangiopancreatography (ERCP) showed normal ductal anatomy. There was no communi‐ cation between the pancreatic duct and the pseu‐ docyst (FIGURE 1D). Endosonography (EUS)‐guided drainage of the mediastinal pancreatic pseudo‐ cyst was performed through the terminal esoph‐ agus using linear EUS. Two passes were made with a 19 ‐gauge needle using a transesophageal approach, and 200 ml of serous amber fluid was removed (FIGURE 1E). High levels of amylase were observed in the fluid (15,774 U/l). Tumor mark‐ ers were within normal ranges. A control CT scan did not confirm the cyst near gastroesophageal hernia, and there was no pathology in the back mediastinum (FIGURE 1F). Pancreatic pseudocysts are diagnosed accidentally in 20% of the patients and on autopsy in 24% of the patients.1 A pseu‐ docyst occurs in 7% to 15% of the patients with Correspondence to: Przemysław Dyrla, MD, PhD, Wojskowy Instytut Medyczny, Klinika Gastroenterologii, ul. Pancreatic pseudocyst is a common complication of acute and chronic pancreatitis. Extrapancreatic locations of pancreatic pseudocyst in the liver, pleura, mediastinum, or pelvis have been described. However, a pancreatic pseudocyst located in the liver is an infrequent condition. We present the case of a 46-year-old man with pancreatic pseudocyst located in the liver secondary to chronic alcoho...

Successful resolution of a mediastinal pseudocyst and pancreatic pleural effusion by endoscopic nasopancreatic drainage.

CONTEXT A mediastinal pseudocyst is an unusual complication of acute and chronic pancreatitis. The ideal form of management is controversial, and various successful therapeutic interventions including surgical resection, internal or external drainage, and non-operative radiological drainage techniques have been described. Successful resolution of a mediastinal pseudocyst with endoscopic transpa...

Mediastinal pseudocyst with pericardial effusion and dysphagia treated by endoscopic drainage.

CONTEXT Pancreatic pseudocysts located in the mediastinum are rare. Symptomatic mediastinal pseudocysts can present with dysphagia, dyspnea, airway obstruction and/or cardiac tamponade. Generally, the standard approaches are surgery and external drainage. Recently, there have been many reports of successful endoscopic drainage mainly using a transpapillary technique. However, there have only be...

Mediastinal extension of a complicated pancreatic pseudocyst; a case report and literature review

BACKGROUND Mediastinal pancreatic pseudocyst is a rare complication of acute or chronic pancreatitis. CASE PRESENTATION This case report describes the management of a difficult case of pancreatic pseudocyst with a mediastinal extension in a patient having chronic pancreatitis. Different management strategies were used until complete resolution of this complex pseudocyst occurred using open su...

Three cases of mediastinal pancreatic pseudocysts

A rare complication of acute or chronic pancreatitis is the formation of a mediastinal pancreatic pseudocyst (MPP), which is caused by tracking of pancreatic fluids through anatomical openings of the diaphragm into the mediastinum. Herein, we report the imaging characteristics of three cases of this condition. Our results revealed three features in common: (i) the connection between the mediast...

Ectopic pancreatic pseudocyst and cyst presenting as a cervical and mediastinal mass - case report and review of the literature

UNLABELLED Ectopic pancreas in the mediastinum is extremely rare. We are reporting on a case of a twenty two year old woman who presented to our clinic with a large cervical mass. The CT scan revealed a cystic lesion in the anterior mediastinum. The patient underwent surgical resection by cervical approach. A Cystic mass with pseudocysts, cysts and complete pancreatic tissue were found in patho...
